Suggestion….
Please use the d/c instructions as much as possible – written- and hand yellow sheet or copy to patient!!
I just finished two more complaints that the d/c sheet would have likely avoided or been my defence /your defence to the patient issue!!
Also – I chart when patent refuses the written d/c sheet which means I offered to write things out for them and they declined!!
It could be a NEW YEARS RESOLUTION!!
I agree, John. Many times I have looked to see what a previous physician has prescribed for the patient and there is nothing documented. Let me tell you, after my CPSO practice review (which resulted in a trip to CPSO offices in Toronto… not fun, trust me), documentation such as the discharge sheet (which was always my practice, thankfully!) is vital. I fill it out 95% of the time, and it takes an extra 60 seconds to do.
